Activity Sheet Feedback Form

As a part of their participation in the I Care program, parents of students are asked to engage in character building and mentoring activities with their children on a monthly basis. Once a month, parents are asked to fill out an activity Feedback Form that gives information on activities the parents engaged in with the child and return the form to their child's school.

"Behold, I will send you Elijah the prophet before the coming of the great and dreadful day of the Lord. And he will turn the hearts of the fathers to the children, and the hearts of the children to the fathers, lest I come and strike the earth with a curse." Malachi 4:5-6
